How do I Register?
- First confirm that you are living within the Practice area.
- Come to the main Surgery and complete the NHS GMS1 form, the Practice Questionnaire and book a New Patient Health Check
- Your Registration will be uploaded to the Local Health Authority.
- Your notes will then be called form your old surgery and sent to us. Please note this can take some time.
How do I make an Appointment?
- For all appointments call 01326-315615
- For Consultation with a Doctor you will be given a Telephone Call Back with the doctor who is currently dealing with you.

How do I get my Test Results?
- The Practice has a “No News is Good News” policy on Results. Only if there is some action required will they contact you.
- If you call in about a result – you will be given a Telephone Call Back with the Doctor who ordered the test.
- The Reception staff can ONLY tell you if the Result has returned and whether the Doctor has seen it. They cannot comment on the result.

How can I order my Prescription?
- You can order your prescription by mail, by dropping in your request, over the phone or on the Prescription Voicemail – the Prescription Number is – 01326 434872
- Repeat prescriptions can be issued without a Telephone Call Back with a doctor and will be ready with 48 hours.
- Acute prescriptions will require a Telephone Call Back with a Doctor and may take up to 5 days to process.

How do I organise my Travel Vaccinations?

How do I know when to attend the local Minor Injuries Unit??
If you have had an accident or suffered an injury within the past 5 days you should attend the Minor Injuries Unit (MIU) at Falmouth Hospital.
The MIU is open 7days a week – from 0800-2000
What do I do if I need a Dentist?
- If you have a dental problem you should contact your own dentist
- If you need Urgent dental care you can contact the dental helpline on 0845 5000230
- If you need medication following a dental procedure you should contact your dentist
How do I see a Doctor Out of Hours?
- The Practice is open from 0800-1800 each weekday
- Outside these times cover is provided by Kernow Urgent Care Services (KUCS)
- To contact KUCS – call the usual Surgery number – 01326 315615 and you will diverted direct to the Out of Hours service
Where can I get medical advice?
- For general medical advice you can contact NHS Direct – 0845 4647
- For information about a medical condition go to www.cks.library.nhs.uk
How do I get transport to Hospital?
- If you are unable to take private or public transport to attend Hospital you can contact Transport Access Patient (TAP) on 01872 223388.
- If you unable for medical reasons to go by private, public or TAP transport the surgery will arrange appropriate ambulance transport, but ONLY for your first outpatient appointment. For all follow up or subsequent appointments the hospital is responsible for arranging the transport.
How do in get a Sick Note?
- For the first 7 days of sickness you can Self Certificate, collect a SC form from the surgery and complete it and pass to your employer
- For any time after the first 7 days you will need to contact your surgery for a Sick Note. You will be given a Telephone Call Back with your doctor
How do I get a Claim Form Completed?
- Many patients will want various reports and forms completed by their doctor. These can range from Fitness to Travel to Holiday Cancellations
- If the request is from a 3rd party the patient must provide written consent
- Please be aware all such reports have a fee
How do I get a Medical Examination?
- There are many types of medical examinations – but in the main they are for employment or insurance. All will have dedicated forms but will have different requirements
- For ALL medical requests from a 3rd Party, Insurance Company, Bank or Employer the patient must have given written consent for us to divulge information. Without this we cannot provide this information
- We will require the associated forms well before we book the Medical
- You will be contacted by our Accounts clerk to arrange a date and time
- Please be aware all such Examinations have a fee - some of which are paid by the Insurance company or employer
