New Registrations

If you wish to Register with our Practice, please first check that you are within our designated Practice area. This area is detailed on the map low by a red dashed line.

If you are within our designated Practice area you simply need to come to the Main Surgery on Trescobeas Road and complete the NHS GMS1 Form and a Practice Health Questionnaire. We will guide you through this when you come in.

Your Registration is then uploaded to the Primary Care Support Agency who will call for your notes from your old Surgery. Please understand that this may take some time.
