Many come out confused about new medicines, unsure about their normal medication, unsure about any follow up requirements or unable to cope alone at home. She works closely with the District Nurses, Social Services and Community Matron all of whom may have some input into the patient’s care.

  • District Nurses. These are Community staff responsible for the care of those patients who are unable to access the surgery due to their medical condition – the housebound and those recently discharged from hospital that need further care at home. The District Nurse team leader is Kim Vickary. They are co-located in the surgery and can be contacted via the surgery or directly on 01326-434885.

  • Health Visitors.  These are community staff responsible for all aspects regarding children under the age of 5 and their families. They hold Child Health Surveillance clinics on Wednesday afternoon most weeks. They can be contacted direct on 01326- 434782.

  • Midwifery service. These are Community staff whose role is to look after our mothers-to-be and arrange all antenatal care. They run Antenatal clinics at the surgery on Wednesdays most weeks.

  • CADA service. This service provides weekly clinics on Tuesdays most weeks. Referral to this service is via your GP.

  • Community Matron. Our Community Matron – Julie Spriggs – is responsible for looking after our most Chronic patients. Her main role is to manage their care to avoid the need for them to go to hospital. She is also responsible for the Home Oxygen provision.

  • Pharmacy. There is a Pharmacy attached to the main surgery at Trescobeas. This allows for a quick and direct service between the practice and the pharmacy.
